It is estimated that half of all men will experience noticeable men hair loss treatment during their lifetime. About 90 percent of cases are genetic, while the rest are due to metabolic factors.

Depending on an individual’s genetic makeup, hair loss genes inherited from either parent may activate at any time after maturity. When activated, hair follicles become sensitive to di-hydro-testosterone (DHT), a testosterone metabolite, leading to progressive miniaturization. Without treatment, these follicles may eventually die, resulting in permanent baldness.

Early activation of these genes, such as in a man’s 20s, can accelerate hair thinning and the overall progression of men’s hair loss compared to later activation.

Genetic Hair Loss and DHT

An enzyme called 5-alpha reductase (5AR) converts testosterone into the hormone Di-Hydro-Testosterone (DHT) in genetic hair loss. This enzyme enters the papilla, outer root sheath, and sebaceous gland cells. Upon binding to androgen receptors, DHT enters the nucleus of these cells to bind with DNA, which in turn produces messenger RNA that causes follicle-damaging proteins to be produced, disrupting the growth cycle of hair.

Hair growth is cut short during the anagen phase, and the hair regresses prematurely to the catagen phase and eventually to the resting state. A new hair grows after the telogen phase with a smaller diameter and a shorter anagen phase.

Progressive miniaturization occurs over and over again, with an ever decreasing anagen growth phase and decreased hair diameter until ultimately the hair is lost.

Metabolic Hair Loss

Metabolic hair loss differs significantly from genetic hair loss. There is no reduction in hair diameter or progressive miniaturization of the hair. Hair loss may begin within a few days of abrupt metabolic changes, though it might start a few weeks or two to three months later. It is normal for 10 percent of the hairs on a scalp to be in the telogen stage at the time of rest, however, in metabolic hair loss, 20 to 50 percent of the follicles are in the telogen stage, resulting in more hair loss. The medical term for this condition is telogen effluvium.

Sometimes, because of damage to the matrix cell in the hair bulb, more than 90% of the hair stops growing. Hair loss characterized by tapered hairs is known as anagen effluvium. Hair loss can be caused by the following metabolic conditions:

  • An imbalance or deficiency in nutrition
  • Drastic weight loss
  • A high level of stress Extreme illness
  • Major surgery
  • Medications such as beta blockers,
  • antidepressants, non-steroidal anti-inflammatory drugs, and chemotherapy
  • Vitamin A intake that is excessive
  • Thyroid dysfunction
